Reviews and evaluates drug regimens of patients in assigned nursing units, and other units as requested. Suggests appropriate, cost-effecitve therapeutic alternatives to prescribers. Provides accurate and timely drug utilization and appropriateness information to physicians, nurses, pharmacists, and other professional staff. Works with appropriate committees and teams to facilitate the development and implementation of clinical practice guidelines, standing order sets, protocols and policies, as requested. Facilitates the identification, resolution, implementation, and communication of cost-effective medication use isues in collaboration with the medical staff. Three (3) to five (5) years experience in hospital clinical setting and MS Pharmacy or Pharm. D. degree, thorough knowledge of drugs and drug therapy; good oral and written communication skills, ability to work with variety of disciplines, ability to multitask and meet deadlines, current licensure or eligibility for licensure in North Carolina as a Pharmacist required.
