School Information

Following your child's diagnosis, you will need to complete a school diabetes care plan packet.  This packet will help you effectively communicate your child's diabetes care to the administration and staff at your child's school.  The packet also clearly outlines specific care responsibilities for both the family and the school. 

Please complete the  school diabetes care plan packet, including the Insulin Dependent - Insulin Pump form.  Have all documents signed by your child's diabetes doctor at your child's next scheduled visit.  Afterward, you will need to meet with your child's school nurse to review the diabetes care plan together and to make sure your child's diabetes supplies are in place.

In an effort to help your child manage his or her carb intake while at school, take a look at the following link to the Wake County Public Schools website, http://www.wcpss.net/child-nutrition/special_diets.html.  The site offers carbohydrate counts for each of the items on the child nutrition menu.
