Microbiology Antimicrobials and Susceptibilities Information

Streptococcus pneumoniae A screen for penicillin susceptibility is performed on all isolates of  Strep pneumoniae at WakeMed. MIC testing by the E-Test method is performed on isolates from Blood and CSF. Note that for Cefotaxime and Ceftriaxone, different MIC breakpoints have been established for meningitis vs. non-meningitis isolates. MIC interpretations are provided for both CSF and non-CSF isolates. In the absence of meningitis, the non-CSF interpretation should be used.

ESBL (Extended Beta Lactamases) ESBL (Extended - Spectrum Beta Lactamases) are enzymes found in gram negative bacilli that mediate resistance to extended-spectrum (third generation) cephalosporins and monobactams.  Confirmatory testing is done for ESBL positive E.coli and Klebsiella sp using the NCCLS ESBL confirmatory disk diffusion method.  In 2003 there were 26 isolates of ESBL positive E. coli and 9 of ESBL Klebsiella pneumoniae.
Antifungal Drugs Most species of Candida are susceptible to Amphotericin B (exception Can. lusitaniae).  Susceptibility to Fluconazole varies with the species of yeast: Candida albicans... Susceptible; Candida krusel and Candida (Torulopsis) glabrata... often Resistant; other species of Candida ...Variable.
Hemophilus influenzae Of 195 isolates in 2003, 75% were beta lactamase negative.  25% were beta lactamase positive (Ampicillin resistant)
